Please utilize the form below to schedule a Free Introductory Consultation with our Groups Coordinator, Dr. Collin Shotts, for Group Therapy services.

Our Groups Coordinator will call you to confirm your interest in our Group Therapy Services, gather some preliminary information, and add you to the waitlist for your selected group. Once we reach the minimum number of members needed, the group will begin.
